Download the resource toolkits

The Medical Advisory Council (MAC) of the Forum of ESRD Networks has developed four toolkits to assist facilities in meeting the Conditions for Coverage. The toolkits address the following topics: medication reconciliation, vaccination, catheter reduction, and assurance of diabetes care coordination. They are available for free download as of August 6, 2009. Please share your comments and/or feedback on the toolkits by clicking the 'Add Comment' button on each toolkit download page. The MAC intends to update these toolkits based on comments from users and would like to have as many comments as possible. If you would like to share your own forms, please include this information in the Comments section as well. Credit will be ascribed to all who submit forms for public use.
