Vaccination Toolkit
This Toolkit for health providers and practitioners is a reference tool that provides information about vaccination requirements for kidney patients in the dialysis facility.
Updated June 2025: The toolkit was amended to include updates on influenza, COVID-19, and Hepatitis B vaccines. The Immunization Data Collection Tool was updated as well.
Updated August 2021: Updates to the Toolkit include a new section on the COVID-19 vaccination. And the Immunization Data Collection Tool has been updated to include the COVID-19 vaccine and the pneumococcal booster + age to track patients >64 for the PPSV-23 booster.
